The Christian Home & Rehabilitation Center is a state licensed, 64-bed Medicare/Medicaid certified Skilled Nursing Facility located in the city of Waupun.  Serving the continuued care needs of area citizens for more than 50 years, the Christian Home & Rehabilitation Center offers both long term care and short term rehabilatative/recuperative care. 

Short Term, Rehabilitative Care

Our 14 bed Transitional Care Unit helps bridge the gap between hospitalization and home offering outstanding rehabilitation services that include Physical, Occupational and Speech therapies.  Individuals recovering from surgery, an elective orthopedic procedure such as a knee or hip replacement, a stroke or other debilitating illnesses, are perfect candidates for continued care in our Transitional Care Unit.  Medicare and  Medical Assistance as well as many commercial insurance, HMO and Medicare Replacement policies cover the care of short term rehabilation at the Christian Home & Rehabilitation Center.  Our knowledgeable staff will assist you in verifying coverage and obtaining authorization for your stay.

Out-Patient Rehab Services

Professional Rehab Services that include Physical, Occupational and Speech therapies are available on an Out-Patient basis.  Our business office and rehab professionals are available to assist individuals to verify coverage of service and obtain authorization for treatment as needed. 

Long Term Care

The Christian Home & Rehabitation Center provides care for individuals requiring daily assistance on a long term basis.  Individuals living here enjoy a comprehensive Activities Program, ongoing Nutrition assessments, onsite Rehabilitation and Social Work services along with a dedicated staff of nurses and caregivers.  Additionally, the Christian Home & Rehabilitation Center has a full-time Chaplain available to residents and their families.

The Christian Home & Rehabiliation Center accepts a variety of payors for long term care including Private Pay, Medical Assistance, Family Care and Long Term Care Insurance Coverage.
